Terms of service.

Rates

Cash pay is a rate of $125.00 per 60-minute individual session. 

Additional cash charges for administrative tasks (file review, copying, consultations with other service providers, etc).

Reduced fee services are available on a limited basis.

Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions when you call your insurance provider (the number is located on the back of your card):

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • Do I need to meet my deductible before I am able to pay a co-pay or reduced fee for my mental health sessions?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is pre-authorization required from my primary care physician?

  • Does my employer provide free EAP (Employee Assistance Program) sessions?

I am a network provider for the following insurance companies:

  • Aetna

  • Anthem

  • Blue Cross Blue Shield 

  • Carelon Behavioral Health

  • Cigna and Evernorth

  • Quest Behavioral Health

Cancellation 

I have a 24-hour cancellation policy. If you cancel within 24 hours, you will be charged a $50 fee. For no-shows or last-minute cancellations (cancelling on or after your appointment time), you will be charged a fee of $100 at the time of your scheduled appointment. Insurance plans will not pay for missed appointments or late cancellations. If you are using Lyra benefits, please note that Lyra does not always cover late cancellations, and you may be responsible for the full session fee out of pocket. Please consult with your employer or Lyra representative for further details.

A late cancellation fee may be waived, at my discretion, in case of a true emergency. Additionally, I waive the fee if we are able to reschedule your appointment within the same calendar week. Please reach out to me directly for any questions regarding the cancellation cutoff period and fees.

No fees will be charged for any changes made to your appointment time with a 24+ hour notice.
